Susan "Sam" Ann Heuvelmans, 61, Green Bay, died Friday, October 16, 2009 at a local hospital after a long illness. She was born August 21, 1948 to Elizabeth (LeCoque) Heuvelmans and the late Roland Heuvelmans. in Green Bay.
Survivors include her mother, Elizabeth "Betty" Heuvelmans; five siblings, Gary Heuvelmans and Tori Cavil, Green Bay; Kevin (Mary), Waukesha; Mary (John) Chapman, Atlanta, Georgia; Larry (Mary Beth), Madison; and Lynne (Ron) Bauman, Atlanta, Georgia; and many nieces and nephews;
She was preceded in death by her father, Roland.
Friends may call after 9 am Monday, October 19, at St. Joseph Catholic Church, 1224 12th Avenue, where a memorial mass will take place at 11 am with the Rev. Donald Everts officiating. Online condolences may be expressed at www.lyndahl.com.
In lieu of flowers, the family requests that memorials may be given to the Susan G. Komen Breast Cancer Foundation in Susan's name.
